Readmission Rates

Why we measure this

MUSC strives to ensure all patients are ready for discharge after a hospital stay, and can remain out of the hospital for at least 30 days. We recognize that some patients will need to come back to the hospital within that time, but strive to get our patients what they need to remain healthy at home.

How we measure this

We measure the percentage of patients that have to come back to be admitted to the hospital within 30 days of being discharged.

These measures are also reported to the Centers for Medicare and Medicaid Services (CMS). CMS offers more information about the readmission rates and comparisons by hospital.

What we do to improve

MUSC strives to ensure all patients are ready for discharge before they leave. A nurse also calls every patient within 72 hours of discharge, to make sure they have what they need and to answer any questions they may have about their health, medications or follow up plan.
