How To Obtain Medical Records via MyChart Patient Portal

MUSC Health offers you an easy way to access your medical records online through MyChart patient portal. Through MyChart, you can easily communicate with your doctor's office, schedule appointments, request prescription refills, review test results and more. If you are a patient of MUSC Health, select the MyChart link to access or create an account. Under the menu tab, click Request Medical Records.

How to Request Medical Records Online (eRequest)

MUSC Health now offers an Online Patient Record Request tool for the following individuals to request patient records:

  • Patients requesting their own records
  • Parents of minor patients requesting records
  • Caregivers acting on behalf of a patient (i.e. Power of Attorney)

Request medical records via secure website hosted by our release of information partner, Ciox. If you are unable to complete the request online, refer to the additional options to submit your medical records request.

The Online tool verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone. If the patient is deceased, please make sure to attach a copy of the Certificate of Appointment as the Personal Representative of the estate.

Manual Request for Records

The Health Information Management department at MUSC Health fulfills requests for information for medical records and billing for all inpatient, outpatient, and clinical services affiliated with MUSC Health. The Release of Information staff will be happy to assist you in obtaining your medical records and/or billing information upon receipt of a valid authorization. You can mail, email, fax, or personally deliver your authorization to the facility location of your choosing, Monday through Friday, 8:00 a.m. to 4:00 p.m.

All requests should be submitted to one of the sites below. Requests should not be submitted to a provider's office or clinic.

Reproductive Rights Attestation

New Attestation Required When Medical Records Are Sought For Health Oversight, Judicial Or Administrative Proceedings, Law Enforcement Purposes, Or Disclosures To Coroner Or Medical Examiners

The US Department of Health and Human Services updated the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule in 2024 to strengthen privacy protections. The updates prohibit covered entities like MUSC Health from disclosing health information when it is potentially related to a patient’s reproductive health and the information is sought for the purpose of identifying an individual for criminal or civil liability, or as part of an administrative investigation.

The Rule requires that MUSC Health and its business associates obtain an attestation when the request is for health oversight activities, judicial or administrative proceedings, law enforcement purposes, and for disclosures to coroners and/or medical examiners. The attestation must have the requestor affirm that the purpose of the use or disclosure of protected health information (PHI) is not to investigate or impose criminal, civil or administrative liability on any person for the mere act of seeking, obtaining, providing, or facilitating lawful reproductive health care or to identify any person for such purposes.

Be advised that MUSC Health will continue to require that a request for PHI be compliant with HIPAA, but will now also require this new attestation for requests associated with health oversight activities, judicial and administrative proceedings, law enforcement purposes and disclosures to coroners and/or medical examiners.

By signing the attestation, the requestor and/or its organization is (1) verifying that they are not requesting PHI for a prohibited purpose, and (2) acknowledging that criminal penalties may apply if their statement is untrue and the PHI is used or disclosed impermissibly.

Please fill out the attestation form (PDF) in its entirety and submit it along with the subpoena, court order or other legally authorized administrative request. Incomplete attestations will not be accepted.

On and after December 23, 2024, unless we receive a signed attestation in each of the requests described above, we will be unable to release the requested records.
