Objectives 1. Describe effective palliative care models in home, telemedicine and outpatient settings
2. Understand the quality and cost outcomes of palliative care models.
3. Discuss how to face outwards towards future needs of our patients and their families, as well as policy makers, payers, and health system leadership.
