Clinical Neurophysiology Education & Training Program Application

Applicants are required to submit the following:

• $50 application fee (see Program Fees)
• Application form (below)
• Transcript(s) indicating degree and prerequisite courses (Anatomy I, Physiology I)
• Resume/CV
• A personal statement expressing why you are interested in this program
• Letter of recommendation
• CPR/BLS Certification
• Completion or scheduling of shadow at MUSC in the desired pathway
