PGY2 Thrombosis & Hemostasis Management Residency

Purpose

PGY2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.

Program Structure

The PGY2 Thrombosis and Hemostasis Management Pharmacy residency program at MUSC is a one-year post-graduate training program designed to prepare clinical pharmacy specialists in advanced training in thrombosis and hemostasis. Residents who complete the program are qualified to accept a position and practice in a variety of thrombosis specialty environments. As part of the extensive training residents will receive, they will be able to identify, prevent, and resolve medication-related problems, participate as active members of a multidisciplinary healthcare team, provide education to various healthcare providers and patients, conduct effective research and medication-use-evaluations, and demonstrate leadership skills.

Adult On-Call

Residents participate in an out-of-house on-call service supporting clinical care for adult patients. There are 2 on-call shifts each week: Weekday (Monday-Friday) and Weekend (Friday-Monday). Weekend shift coverage includes an on-site weekend shift for Saturday and Sunday; otherwise, on-call can be facilitated remotely. There is always a clinical pharmacy specialist to support residents and work together to find appropriate clinical solutions. One resident covers on-call at a time.

For 2024-2025 residents, six shifts were scheduled to each resident:

  • Three weekday fully remote from Monday to Thursday
  • Three weekend shifts from Friday to Monday with on-site clinical shift 0700 to 1530 on Saturday and Sunday
  • Total of 21 days on-call for the year

  4. Ambulatory LVAD Clinic (1 month)
  5. Anticoagulation Stewardship (1 month)
  6. Benign Hematology Consult Service (1 month)
  7. Medical Intensive Care Unit (MICU; 1 month)
  8. Cardiothoracic Intensive Care Unit (CTICU; 1 month)
  9. Neurocritical Care (1 month)
  10. Research Project Month (1 month, December)

Elective Learning Experiences

  • Heart/Lung Transplant (1 month)
  • Medication Use Safety/Industry (1 month)
  • Repeat any required rotations (1 month)

Longitudinal Learning Experiences

  • Adult Inpatient On-Call (evaluated 6 times annually)
  • Anticoagulation Stewardship (yearlong)
  • Management/Leadership (yearlong)
  • Medication Use Evaluation (MUE; 6 months)
  • Research Project (yearlong)
  • Staffing Experience (yearlong)

Additional Program-Specific Completion Requirements

  1. Up to 26, 8-hour weekend shifts (average of every 4th weekend over 52 weeks)
  2. Up to days 26 days on-call
  3. Present at one Anticoagulation Forum Lecture
  4. Journal club presentations – 4 required per quarter
  5. Research Certificate required (if not completed already)
  6. Complete one committee assignment (institutional, local, or national)
  7. Revise or develop one guideline/protocol
