Diabetes Prevention Program

The SHINE program is based on the CDC's Prevent T2 Curriculum with the primary goal for all participants to maintain 150 minutes of exercise each week and lose 5% to 9% body weight.

We are currently enrolling participants for new cohorts!

  • When: Weekly meetings for the first 4-6 months (with breaks for major holidays) and monthly meetings thereafter for a total of 1 year
  • Where: Online using the Zoom platform (accessible from any mobile device or computer with Internet access)
  • Cost: covered at no cost by Medicare Part B and some Medicare Advantage plans, or self-pay ($351/year)
  • Eligibility: Adults (over age 18) at risk of developing type 2 diabetes may be able to participate in this program, including those with a diagnosis of prediabetes.
  • You may have prediabetes, if you have a blood test result in the past year with 1 of the following results: Fasting plasma glucose of 100-125 mg/dL, Hemoglobin A1c (HbA1c) of 5.7 to 6.4%, or 2-hour Plasma Glucose of 140-199 mg/dL. Please note: for Medicare to cover the cost of the program, your blood test result for fasting plasma glucose must be in the range of 110-125 mg/dL.
  • Additionally, participants must have a body mass index (BMI) above 25 kg/m2 (or above 23 kg/m2 if Asian) and NO history of end stage renal disease or diagnosis of type 1 or 2 diabetes
